I'd only use subtle stuff for that.

The TCG symbols always looks pretty nice and only people that actually play Pokémon will know what it is.

Patients don't even care, and if they do they are not worth your time.
If they seriously judge you on what you own instead of how well you perform as a doctor, they are the ones at fault.

He's right though.

If a patient notices that a doc isn't completely serious all the time they'll think that you're incompetent.
They might not say it out loud but they believe it.

If I saw my doctor with a Pokemon phone case I'd assume he's some sort of mad scientist who will try to use experimental drugs on me to try and make artificial Pokemon.

In all seriousness, I don't think people would judge you for it anymore than they would for having Pokemon merch period. That being said, people will probably judge you for it period.

not for my phone, but I got the whimsicott 3DS case last week. It's nice and subtle - you'd only recognise the substitute if you play the games, and the whimsicott in the corner doesn't look like anything to people who only recognise gen 1-2 mons

You only have to seem professional in front of patients or bosses, if you've ever talked to a surgeon you'll find they have very dark senses of humour, along the lines of "well the good news is he won't have to worry about X" when discussing a terminal case with his peers.
