For the players that have a complete Living Dex or are working on one: did you include different forms in your collection?

Right now, my Living Dex just has one of each 'mon, excluding forms. Was just thinking to go that extra mile and gather all the different forms as well.

To dictate whether to include the form or not, I was going to see if it had a unique icon? Or is there something else I should go by?
