We get to see what Johto was like three years before the events of G/S/C, including a somewhat different stable of Gym Leaders and typings (i.e, we get to see Jasmine as a Rock Leader, Karen could be the first ever Dark Leader).
Other than that:
- Token extra batch of Mega Evolutions, this time focused on Gen I and Gen II mons and their cross-gen Evolutions. Venusaur and Blastoise could get secondary Megas like Charizard, and there could even be Megas for the Johto Starters.
- Mewtwo could get its own version of the Delta Episode, tying into the Sevii Islands thing from FR/LG's postgame. Replace Team Rocket trying to steal the Ruby and Sapphire with trying to capture Mewtwo.
- Mew and Celebi get downgraded from 'mythical' status like Deoxys was in the Delta Episode; they can be captured after you beat the Johto Leaders and complete the Kanto and Johto Pokedexes, respectively. Mew can be captured on top of Mt Silver, Celebi in Ilex Forest.
- Ho-Oh and Lugia get Primal Reversions; Lugia/Primal Lugia is exclusive to the Red remake, Ho-Oh/Primal Ho-Oh is exclusive to the Blue/Green remake. They can be captured on Navel Rock, which is now a normal part of the game in the Sevii Islands area.
