Many, many times, actually. Often times it happened when I didn't read the log to notice a pokemon had already used their Focus Sash, or I mispredicted because I completely forgot that I already killed a specific pokemon on the enemy team. Stupid shit like that of me being lazy.
It was especially bad because back when I played competitive OU during 5th gen I had a highly defensive, counter-based team which heavily relied on my own often correct predictions. So it often won, except those times I made a stupid move which lost me the match or when the opponent crit haxed me.
