there are now three routes and two towns on the way to mt silver. The first town is a rich people resort town, the second town has a wild-west feel with trainers preparing to take on mt. silver or trying to forget how badly they failed
Route 11 is now full of schoolkids and teachers out on a field trip. Its expanded to take advantage of all that space to the south, maybe another forest?
Cinnabar is 3 large islands, one is the volcano, one is the city and one is wilderness with a research center. The three islands are close together and have bridges
Digletts cave gets another lower level, accessible near both entrances. It's the Kanto Underground, which spans most of the region. Its full of trainers and items and gets an expansion post E4 with national dex pokemon and fossils.
Dive spots around the east coast, lots of coral and kelp. There with be a building somewhere that will let you take scuba trips
Power plant is a nuclear plant that is up and running. There's a new section of rock tunnel with unnova style electrified rocks, where everything formerly found in the plant now lives.
Post E-4, Viridian forest catches fire, which changes the appearance and opens a new area that connects to victory road. Side quest to find the pokemon that caused it, it's a baby (rare fire pokemon, larvesta maybe?)
Bill has a garden, but everything is withering and dying. He admits to not having a green thumb, and mumbles something about shaymin. Triggers a shaymin event

Route 16's bike route is raised and separated from route 16, like hoenn's
That funky little peninsula by route 12 and 13 is accessible and is one of those sweet as fuck cliff areas that gen 5 had
