The progression of the game is pretty much the same as in RBY up until Pewter, after which the game becomes largely non-linear. All of the primary locations from RBY/FRLG are back, with bigger, more diverse environments, overhauls of the cities, and a couple of new locations (a proper Wind Energy plant on Route 14 for example, a volcano dungeon on Cinnabar). The Kanto Pokedex is expanded to have at least 150 more Pokemon.
Once you complete the game, you get to go to the Sevii Islands and complete the Sevii League - each of the islands has a gym on it, though it isn't necessarily in the town/settlement areas. A new eighth island exists (for the sake of an eighth gym), with the Sevii Elite Four (Lorelei as the champion?) residing in a new facility on Navel Rock. Once again, the Sevii league is largely non-linear and can be completed in many different orders.
Oh, and throw in some fanwank. The girl near Mt. Moon should be grown up and have a Munna in her team now. The guy in Vermillion City should finish his building, which is a battle facility. Shaymin will be catchable in Kanto, just like how Oak mentioned back in generation 4
