Keep in mind this is just some random picture, and a definitive set of needs probably doesn't exist. It's merely a framework, a guideline. And it was developed in the 1940's, so don't think psychologists are treating it like the DSM.
To answer your question, you won't feel fulfilled until you've hit at least *some* of the self-actualization listed at the top of the pyramid, and missing too many things from previous tiers will make it difficult or impossible to achieve that. Nobody gives a fuck about creativity or self-esteem when their safety is at risk.
