There were megas such as Gengar, Blaziken, and Kangaskhan at the start that were clearly too much for the tier, naturally they were given priority when banning.
Next was Swagger, which was deemed uncompetitive and annoying for obvious reasons.
Baton Pass was prevalent on ladder, and was also deemed uncompetitive because it was formulaic and guaranteed wins against certain playstyles. It was nerfed twice, after limiting it to 3 mons was shown to be ineffective.
Afterwards, another playstyle was given attention; Deoxys Hazard Stacking teams. Easy hazards in combination with Bisharp and a ghost to keep them up was incredibly powerful, and was dominant on the ladder until Deoxys was banned.
Aegislash centered the meta around itself, it was on most teams and made other threats very powerful as a result. It was a split vote, and some considered it unfair because the vote changed last second in the pro-ban side's favor.
The final suspect of XY was Mawile. It was around this time that the SubPunch set was discovered, which screwed over traditional checks like Heatran. Its raw power and ability to demolish its checks with SubPunch led to its supermajority ban.
There was a Victory Road thread about potential suspects, the main two being Charizard X and Thundurus but Greninja was also mentioned. I can't find the VR forum anymore, if someone could link it I'd be grateful.
