You start in Pallet, go through Kanto, beat the Pokemon League which features Blue as Champion, then you go to Johto through Tohjo Falls, beat the 8 Johto leaders, there is a separate Johto Elite 4 and Champion potentially located at Mt Silver but you could certainly pick another location in Johto, Lance is still Champion of Johto but you could have Silver as head of the E4, then you can climb Mt Silver to face Red, after you've defeated Red he returns to Kanto and takes up the Champion position for rematches with Blue moving down to head of the E4.
Obviously this is unrealistic but it would definitely be the greatest Pokemon game of all-time if done well.
