The same people that don't go to the dentist are the same people that the meme is targeting.
Americans go to the dentist more often, because they want perfect white teeth. They're more likely to get fillings, caps, surgeries, and the like done to them if it means they get perfect teeth. Most people in other countries do not care about that as much as americans do.
