Most fairies are bulky enough to tank a non-STAB Steel or Poison move, especially if they're special, since most are built with that level of bulk. Not to mention that Dragons would rather run both Ground and Fire coverage over Poison/Steel for a reason. Poison/Steel are weak offensive types, and are way too situational because they're only useful against fairies. Meanwhile Ground and Fire at the same time allow dragons to hurt a lot more opponents, and a larger variety. Ultimately running Iron Head is sacrificing a moveslot for the purposes of hitting fairies, when you can just run better coverage and hit more things, which accepting your BTFO from fairies on your dragon and having an actual Steel or Poison type in the back to counter them.
The trade off is much more in favor of running wider coverage to hit more types of mons, than just running a Sterl move, which is really only situational against Fairies. That kind of coverage to hurt fairies, ironically, leaves you with more counters and more that can hurt you, because you're running an inferior offensive type on it. Hence why most (good) Dragon types go for Dragon/Fire/Ground coverage.
