Yeah Kanto, Johto, Hoenn, and Sinnoh are all one country. Oak states this in Gen 4.

Unova is in a separate country that Alola is also likely part of, given that they're both based on US states (New York and Hawaii, respectively).

Kalos, being based on France, is in the Pokemon world's equivalent to Europe.

I think the "National" Pokedex was coined from the fact that Kanto and Johto were two regions/countries sharing a country/continent in the same way as real-life ones.
