It's depicting champions as adversaries. Red is only listed because he's the only champion who has served as an adversary for the player. Besides some BW2 thing I think? Haven't played it, but credits roll after defeating Red, so it's more main-game than the postgame fights in other generations.

I'm still not really sure who was champion before Blue. Was the position empty like Generations suggested? Lance was the leader of the elite four, wouldn't he be champion if the position was open? You could argue he was already, but doesn't the champion have the elite FOUR under him/her?

You can fight the E4 and champion as many times as you want in postgame so it's assumed you also refuse the title I guess.

Are Johto and Kanto leagues supposed to be different? I mean, both Lance and Bruno are in the E4/ champion in both games and even Koga joins in the second.

I believe the league was established fairly new and just no one has been able to challenge it and win before blue. The champion spot was reserved for the trainer who beat the elite four, blue just so happened to be the first
