How to describe it and what is the appeal? Well, Vaporeon can change its molecular structure up to melt into water, according to many of its pokedex entries. So changing up some of its body to make itself malleable, and letting its trainer reach into its body and touch it in unique ways is quite interesting. Since there's no story attached (and I can't read japanese), imagining up specifics fills in for the rest.
For instance, Vaporeon looks relieved. Maybe melting like that is relieving in the same way as undressing and sprawling out in bed at the end of a long day. Its trainer (or lover) is just helping it off of that bothersome bony frame. Intimate and pleasing, but very unusual and worrying to those on the outside, which makes it all the more satisfying from within.
Alternately, the trainer could be harmlessly taking out the vaporeon's spine to temporarily disable it, with intent to put it back later, as a sort of unusual BDSM play. A kind only very unique anatomy allows for. Maybe it's just teasing?
We can't know the details, but we can know of four things: consent, pleasure, (very likely) survival, and manipulation of a very deep, core part of a person's body. Beyond that, imagination is your dick's best friend.
