The wrong kind of underwear actually increases the odds of contracting something from trapping all that Alolan humidity and sweat from walking everywhere.
The real reason she'd be wearing underwear is it suits her character - she already seems to jump at the drop of a hat, so she'd be taking every precaution against making herself embarrassed.
