Prism is a Gen 2 hack with two full regions (a "new" region called Naljo and an "old" region called Rijon, Rijon being the region from the 2004 ROMhack Brown by the same person who made Prism), character customization, Pokémon from Gen 3/4 (plus Sylveon and a handful of Fakemon), abilities, the phys/spec split, and a bunch of tiny new features like mining for stones and crafting balls from Apricorns, sections where you play as your Pokémon, etc.
The hack's been in development hell since 2007, finally got completed when a third party (TPP) stepped in to help with development, got hyped to hell and back by TPP, and then became the first ROMhack to ever get C&D'd (four days before public release) by Nintendo because all the hype ended up on their radar.
A leaked version of the game got released by an anonymous source, but it's missing a few postgame sidequests and has some bugs of varying severity. It's still playable from start to finish though, and there's an unofficial patch called the "Carl patch" that fixes a few of the bugs.
