Typically, I found that most people who had it tended to focus on the helpless aspect of it, and a sizable portion weren't even in to the guro aspect.
Now, I only had personal testimony to work with, and it's possible that even if this trend is bunk, amputation is an outlier, and it still doesn't explain a few of the others who gave other reasons, it did weakly confirm that there actually was an underlying cause. I'd be willing to bet (not a whole bunch, maybe like four bucks) that most fetishes to actually have a root cause (Though what we group under one fetish may actually be made up of sub-varieties, such as the people into amputation for the helplessness aspect, and the couple who were just interested in fucking something up as much as possible).
But fuck it. It's too damn late for this shit and I'm rambling.
