Johto is where Osaka is, Kanto is the other half of the "mainland" with Tokyo.
Sinnoh is Hokkaido.
And Hoenn is that island right to the SW of the main island.... the one that's basically connected but not. (I can never remember the name.)
Johto and Hoenn are literally right next to each other. No farther than Kanto is to Johto. Or Sinnoh is to Kanto.
