It looks like they're reusing route 1
They show in the trailer the MC in the same room that you have in SM, same with the house, so unless you start at your house and then immediately go to take a boat and start on a new island, they're reusing route 1.

man, now that i look at the map, it looks weird how empty it is, it looks like you could easily put 2 more whole isles

They're not new protagonists, they're redesigned protagonists. And they're meant to be generic templates to customize anyway so it's not like they're real characters.
