Every time you went to a new region you'd have a certain level cap that restricted you from using your strongest Pokemon until after you hit a certain flagpoint, most likely the 8th gym leader.
And then handle legendary Pokémon like Pokemon Go but not as retarded. Legendaries would appear as raid battles and they can randomly flee. They are only catcheable by the top trainers who did the most damage.
Then there'd be battle facilities and locations for general PVP and PVE like the Battle Frontier, etc.
Pokémon League would be an annual thing, held every few months, and only the top 4 trainers can go on to the Elite 4, and whoever is left standing battle it out to find out who goes on to become Champion. If they lose against the Champion, they get a 2nd chance via my final point
Title Defense would carry on from the main games. Only a certain few trainers that have met the special requirements are eligible to fight the Champion of a region without having to go through the League.
Of course, the chances of this ever happening is 0. Cool to think about though.
