Kanto is the region name in both Japan and the west
Same with Johto
Same with Hoenn
Same with Sinnoh
Same with Kalos
Same with Alola

Unova is Isshu in Japan. Why did they change it this one time?

Because "Isshu" is derived from a Japanese word.

The region itself is based on the USA, so it makes sense to give it a different name in the international release.

Because why would you want the first western region name to sound blatantly eastern? If anything this is a rare instance where the localization made the better choice than the original Japanese.
