There's a Japanese site that has info how the Pokemon in Red/Green were organized in the game data, and it has some info on which Pokemon were designed by who, which got me a couple more.
That same page says the they heard that an early book stated that Nishida also designed Slowbro and Exeggutor, but they couldn't confirm it. So I left that out.
A couple of Morimoto's Pokemon - Diglett and Mankey - I added based on dialogue I was able to find of what his avatar in SM says if you talk to him with Pokemon transferred from the VC version. I'm not sure if that means Sugimori designing Diglett is wrong or not, so I just placed it on both.
I did checking through individual Bulbapedia pages for the designers to see if I saw anything new or any sources. The page on Hironobu Yoshida claimed he designed Rayquaza, but that didn't have a source like the other Pokemon on his page, and I couldn't find anything about it, so that seemed suspect and I left it out.
When checking through the interviews on Gen V Pokemon linked as sources, I did see some interesting information. I left them in on Unno's line because we don't know who designed them exactly, but Unno states that someone else actually finished up designing the three monkey's evolutions after he made the unevolved forms.
Also, the same person designed Foogus, Amoonguss, Ferroseed, and Ferrothorn, but they didn't say who.
And Sugimori was in charge of Tonadus, Thundurus, and Landorus, but the way they put it, someone else actually designed them fully, and Sugimori just gave directions to the designer. It's arguable if that counts for putting the genies down for Sugimori, so I left it out.
