How hard would it be to create a ROM hack that updates Pokémon Black Version 2 and Pokémon White Version 2 to the state of the new games (new Pokémon, Fairy type, Mega Evolution, Z-Moves, etc.)?

Extremely hard. New Pokemon and Fairy type would be the easiest parts and those aren't exactly trivial to add. Just wait ten years for it to be disassembled.

It would be easier to create a whole new engine than it would be to reverse engineer the DS games to such a state that you could do that.

excluding the z-moves and new pokemon/forms you can get the rest done with the pokemon randomizer, as well as add physical/special split for any game gen 3 or earlier

Yeah, it requires a lot more technical skill than adding new areas. The DS games still use tiles in most places, they're just layered onto 3D maps, and you could always use the existing tiles on new 3D models. On the other hand, only two people have ever expanded the Pokedex in a DS game, and it's pretty advanced stuff. Mega evolution and Z-moves would be even harder if you wanted proper animations for them.
