A starter can be gifted, hatched, befriended, or caught. Any sort of authority figure, ranging from parents to gym leaders to Nurse Joys to professors, can help a kid get their first Pokemon, and they can also do it on their own to an extent.
Apparently, according to the anime, there are secret starter breeding grounds where each traditional starter is bred and raised en masse, but that's just dumb.
