I am making a hackrom about the Sinnoh region, 20 years after the events of Pokémon Platinum, what changes do you recommend doing in the history / mapping?

Cyrus is MIA unless you bring him back. Giratina is probably in the Distorted World again. The lakes are restored and the weather is back to normal.
Being set 20 years in the future puts it ahead of pretty much everything else, so you might be able to get away with adding whatever pokemon from other regions under the pretense of conservation issues, like what happened in B2W2 with the transporter mishap.
Depending on what you want the story to be like, you can include events in Sinjoh or the Battle Resort. Take the player to Full Moon / New Moon Island or the Flower Path during the story. Repurpose buildings like what gen 2 did to the Pokemon Tower in Lavender Town. Shut down the Fuego Ironworks and fill it with poison mons because there's nobody left to clean up the remaining waste. Have miners excavate caves in the Underground that lead to previously unknown ruins. Add a train to Kanto if you want.
There's a lot that you can do.

Oreburgh mines further developed more south and deeper.
Twinleaf expand a little around that pond? Perhaps due to fame of a certain trainer whom hailed from there.
How about an Iceberg laden route going eastwards from Snowpoint's Harbour? Can have seel pokemon chilling about.
What if Stark Mountain erupted in the past 20 years that altered that island's geography a bit? Maybe survival area is now toast.
Can you make Pal Park a full fledged Safari Zone? I know the Great Marsh exists but it is a bit different.
Speaking of swamps, please no change. I may be in the minority here but I like the boginess of it all, reminds me of home. Slow going, humid, and beautiful. Full of bugs too.
What are your plans for an evil team? Some Galactic loyalists?

I'm in for no changes on the swamps, but the way Pastoria's marsh works needs a major overhaul, the whole mechanic of not being able to check which pokemon are in which specific zones because of the zoom the binoculars make, getting stuck on mud and the rest made it insufferable last time I played platinum about 3 months ago.

It is not something defended, it is a jubilife city prototype, because the amount of tiles in gba is quite limited I decided to do it that way, with the explanation that shaymin made the city flourish and the inhabitants found it more colorful, although as I said it's just a prototype, I'll probably change it, I was thinking about focusing the story with the third silvally, arceus and a renewed galaxy team
