The project of including all of Kanto wasn't realistic. This is a common thing in game design: a near idea that is found to be unreasonable early in the development and then devs have to cut down stuff until it's reasonable. This happens all the time. Deus Ex was supposed to have like 200 missions in total, but it just was unfeasible.
That map wouldn't have fit on a GBC cartridge. It would've been barren and severely cut down anyway.
They took the second best decision of including the full map of the first game and expand it.
