The mainland would be pretty large; at least 3-4 "cities", another 6-9 towns, and another 5-7 small, unincorporated rural villages.
The island would be larger than any of the islands from Sun and Moon; 1 "city" at least 3-4 towns, one beachfront, Slateport-like town/industrial center, and two or three smaller villages.
The smaller islands, of which there would be, like, 3-4 would consist of one small town, and a walkabout. Bigger than Cinnabar, but not by much
Maybe instead of an island, one of 'em can be an intact New Mauville/Oil Rig or something.
The mainland would be largely divided into thirds, with a Northwestern, Central/Southwestern, and eastern area. The Northwestern area would be higher elevation, mountainous desert with deep caves. At least one large town or city is completely underground, save for the Pokemon Center you Fly to that is at the top of a lift. There's a tunnel to one of the smaller islands.
The central area would be a swampy bayou in the southwest, grading more into foresty wildland in the central south, then becoming much more amicably climated towards the central and northern central area. The East has a bigass lake, with lots of wide open spaces.
The big island would mostly be jungly and marshy, at least one of the smaller islands would have a volcano.
The region would be semi-open world, with a total of at least 12 gyms. You only need to take 8 Badges, but you are free to take the rest if you want. The sub-regions of the mainland and the islands would open up after certain story marks were met, like, Badge 1 is always the same, or you have to beat Team Dickwad in the eastern mountain pass to access the eastern area, and they are a static level 30 or so.
