Since Totem battles appear to be one of the better received things from Gen 7, how would you improve upon it in the future if they were to ever return?

I think they should keep them but. Ot fully replace all gyms.
Something like optional quests that reward you with items/rare Pokémon eggs sounds alright.

Make them side content. Stronger optional bosses in the routes. Pretty much like unique monsters in Xenoblade.
