I can't prove this happened at all, but here we go.
I got a GameShark for Christmas that I used with Emerald. I'd realized that by selling the Sea Incense, I'd forever squandered my opportunity to get Azurill (DP were already out so trading wasn't a possibility) until now. So I picked one up from the cheat shop, bred me an Azurill, and went back to the PC in Dewford Town to deposit it.
Being a small child, I'd bred a bunch of Magikarp for no reason after getting access to the daycare. I think I had 5 of them. I nicknamed the first one "M-G JR." because it was the child of what would potentially be both a Magikarp and a Gyarados. In my infinite childlike imagination, I nicknamed the other four "M-G JR. 2" through "M-G JR. 5."
They'd all been forgotten in the PC, forever doomed to sit there at Level 5 as I went off to face the league with my four-HM-move Swampert and five Victory Road trashmons because I didn't train anyone else, and they would've stayed forgotten if, when I went to drop off Azurill, they were all still Level 5 Magikarp.
They weren't.
"M-G JR. 2" had somehow gone from a Level 5 Magikarp to a Level 0 Mewtwo.
Seriously.
I have zero closure on HOW this happened other than that whatever cocktail of antipiracy and GameShark code was turning my PC mons into Bad Eggs fucked up big-time, but M-G JR. 2's only move was a jumble of glitchy non-characters in the third move slot. When I used it, the game had this to say:
>M-G JR. 2 used (illegible)!
>?????????????????
>!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!
>M-G JR. 2's attack missed!
>Wild WINGULL used WING ATTACK!
>M-G JR. 2 fainted!
After that, I fed my level 0 Mewtwo rare candies and it learned actual moves and forgot its weird fuckmove once it hit level 1. I re-christened it "Mewthree" with clever use of the "Access Birch's Bag" cheat (since I'd used cheats to rename myself and the game no longer recognized me as M-G JR. 2's trainer) and got Mewtwo regged as seen in the Dex by making Mewthree a contest star.
