Not much changed from Gen 1. 100 new Pokémon, 2 new types, a few new moves, and a different story in a different map. You can return to Kanto and experience a lot of the old stuff.

Johto is the main game and a separate entity. Kanto is accessible after beating the Elite 4, which is where Kanto ended. Much of Kanto has changed.
