In SM, Lusamine's actions are explainable by insanity, brought about by loss of her husband, developing an obsession whether it be trying to get him back or simply the papers he left behind, and potential exposure to Ultra Space as well as Nihilego venom. While it's never stated that she was infected, there is certainly plenty of opportunity for exposure in her history before the player meets her, and she exhibits symptoms of it that match what is described in Nihilego's Pokedex entries. In her deranged state, she has extremely specific visions of what "beauty" and "perfection" are; she emotionally and verbally abuses her children, kicks people while they're down to advance herself or just to break them, and has almost single-minded in getting Nihilego.
Then you have USUM: Nihilego is dropped entirely after the player beats it at Aether Paradise. She's not a lunatic anymore (supposedly), and entirely accountable for her actions. However, GameFreak toned down the things she says, but not what she DOES. She still tortures Cosmog, still makes Type: Null, still cryogenically freezes things, still dresses her kids how she wants (causing a plothole with Lillie), still disowns her kids, and still is willing to play with others' lives. Everyone else is just "okay" with it, because she's doing it for a "good cause," beating Necrozma.
