Irl you have many women who are short and/or flat, and look way younger than they really are (for example a woman who goes to my french classes who's 30 but looks like 17, I still find it hard to believe but it's true). Then you have cases like my sister, when she got her period she was only 10 years old, and because of that she developed HUGE tits which made her feel incredibly uncomfortable among her friends that still looked like, well, little girls. But huge tits or not, she still looked like a child, same way the french classes girl is very mature, but since anime has that sameface thing going on and everyone looks pretty much the same body wise and everything, no matter the age everyone will still look young. So to someone unfamiliar with X character, it'll most likely look like loli to them.
Irl you have different cues to estimate someone's age, but drawn it can be very hard unless some features are exaggerated, which many artists choose to do even if that means sacrificing accuracy, because otherwise it'd honestly just look like loli. Same thing with boy characters, really.
