Iwata single-handedly fit both Kanto and Johto in the same cartridge. Ever since Iwata move away from Pokemon to become Nintendo director, GF can't even handle 1 region properly

Johto was made being a sequel in mind. That's why it's expanding on Kanto with gym leaders, pokemon and region. After that it became a series and every new region can be based on, more distinct other place on Earth.
