It's not only Duplica's Ditto, but it's not all Ditto either.
Some Ditto are too stupid to transform correctly, so they do it with imperfections, like the :] face

Isn't it like, if the Ditto remembers it clearly or has seen it recently, it transforms into it perfectly. But, if it doesn't remember well, it will create flaws such as not changing the face?

According to the Pokedex the quality of transformations varies from Ditto to Ditto so there are plenty of others like Duplica’s who can’t do the faces right.
