I tried doing a no-items-during-battles run with Soul Silver, and holy shit the fucking Elder and Falkner are retarded unless you grind or explicitly have a Pokemon to counter flying.

Depends on the game, really.
I mix up what my secondary restrictions or other allowances are for a nuzlocke all the time.
For my blind run of Y and Sun, for example, I instated a "gotta have it" rule, where I could ignore the usual capture restrictions 3 times per run if I saw something new I just had to have. I had to declare it the first time I saw the pokemon, and if the pokemon was owned by a trainer, it stuck until the first time I encountered a wild one, or something I thought evolved into it.

I did a different run of Y where amie was allowed, but not mid-battle items, I did a SS run where I couldn't grind (I had my own honor system, I defeat Pokemon as normal while getting through a place, but no dawdling for battles, and if I have to go back, I have to run from everything)
My AS run, besides the first and last gyms, I had to box the surviving party I used for a gym leader, and could only get them back after the next leader went down, limited my grinding, but used the EXP Share.

Do what feels fun, and don't be afraid to change the rules mid-game.

Up to you really. I used items in my last nuzlocke. Within reason; if you're just repeatedly healing to stall out PP then I feel that invalidates the point of doing a challenge, but I find that something like a well placed X item here and there is more fun than grinding for those stat points. You can limit the amount of items used per battle as well for a middle ground.

I like doing this one, it requires a bit of research/flexibility, but only using as many items as the other trainer uses is one I like doing. This means you can match Full Restore Super Soakers with Gen 2 Lance, for example.
