Give basic trainers the gym leader AI, gym leaders get the E4 AI, and the E4 get Battle Tower AI. Switch up the movesets and teams, and give most notable trainers an extra Pokémon. When I was into modding gen 3, it was astounding how easy it was to make the game more challenging

Semi-scaling levels plus choosing a starting town. Like, for instance, you could chose the "easy" path by starting in the woods, and the "hard" by starting in the snowy village. In-game teams and wild pokemon will scale their levels based on this path

90% of pokemon difficulty comes from giving the AI options to things the player cannot have (yet). So better/hacked movesets and stats would go a long way towards increasing or decreasing the difficulty.

It's really difficult to do for Pokemon. The in-game trainers are supposed to be pretty brain-dead. It's better that way. ROMs that have difficulty increases always feel cheap, just like when a fighting game reads your inputs.
