Several years ago we had a very popular show where a sexologist would explain stuff and give advice to people. One time a woman called saying she used the vacuum directly on top of her vulva, thinking that it'd emulate a "licking" sensation, but only harmed her in the process. She asked how to "use the vacuum properly", and the sexologist just stared silently into the camera with utter shock and eventually was like "No.. no, don't, no. Don't do that. No."
