Sabrina would probably have a more normal outfit instead of the red suit.
I do have a hard time imagining when and where they would introduce her character. The spearow/Pikachu thing is pretty important and putting a Psychic kid there is way too distracting. I suppose she could be more aloof/surprised at her bike being stolen if you replace her with Misty directly, but it seems more fitting to be introduced later, maybe even as late as Mt. Moon, but more likely she has a Pokemon stolen by Team Rocket or is lost or something in the Viridian area.
More butterfly effect stuff-
Misty might be shifted to be a little older and a more serious trainer, the sisters of Cerulean city might not exist.
Since she's not a water trainer and there are few psychic Pokemon, more random Pokemon might get highlighted or added to her team like Clefairy, Exeggcute, etc.
It might be a secret that Sabrina is a gym leader until they get to Saffron city, and since that's longer it's a bigger surprise. The gym might be run by a made up father character or something that wants her to train harder or return to the gym when she doesn't want to.
Alternatively the fighting gym might replace her spot and tie into Ash's Primeape, and the relatively random fighting trainer from the original show. Might also tie into a younger Sabrina's dream of becoming a gym leader.
The first Pokemon movie would have a strange dynamic between Sabrina and Mewtwo.
Many more kids would be interested in the occult and there might be more backlash to Pokemon in general if psychic stuff was a prominent focus instead of a background element. If Abra was evolved to Kadabra at any point the Uri Geller lawsuit might have been more damaging.
Finally with Misty gone and the doll house episode along with it, thousands of people have their subliminal tomboy/redhead/giant fetish replaced with hypnosis/mind control/brunette/aloof girl fetish.
