(1) the routes actually had enough trainers to wear down the player
(2) Trainers didn't give several thousand Pokedollars after every battle making money basically pointless outside of cosmetics
(3) They were entirely optional
The reason why the lady on Route 111 in Hoenn made sense was because you had to go through two Routes and a mini-dungeon to get to that point and then had another Route to go through after that before you go to your next town, that was also at a fairly early stage in the game where the game is still not quite expecting the player to be fully prepared, the one they really didn't need to have was the Nurse near Mt. Pyre since you're close enough to the Safari Zone and Lilycove City that you can just heal there instead.
