I'd imagine it's the idea that these plague doctors essentially have the freedom to do whatever they want to the patient, since nobody else is willing to get anywhere near them due to fear of contracting the plague. Imagine the girl, shivering, covered in sweat, feeling like she's about to die and being completely unable to resist the plague doctors' advances. Or conversely, imagine being the doctors. Yet another patient who will likely die regardless of your efforts to cure her. You gaze down at her delicate, naked form knowing full well if you so much as took your mask off in her vicinity, you'll likely end up in her shoes. But nonetheless you cannot help but feel some level of titillation from her budding, prepubescent body. It puts you in the perspective of both parties and elicits many different emotions. Art is not always meant to be purely pleasurable. Deriving negative emotions from a work can be just as poignant as feelings of jubilation or arousal. This piece in particular is very evocative due to its inherently disturbing atmosphere; at a moment's glance the first thing that pops into your head is "WTF?".
