But it kind of is. So many of the leaders and trainers use old Pokemon that it feels like Kanto 1.5

They are the same and not the same, that is the problem. The structure in general is the same but the execution and style is different. Hence bias.
It's like Sonic fans arguing that Sonic Adventure 1 is better than 2 despite both being good and the only real difference being how the levels are structured and the plot is different. (Ok this might be a bad example my bad)

Johto only looks small on the surface. In terms of overall area including dungeons/forests/towers, gen 2 and especially the gen 4 remakes of them are pretty substantial compared to the literal skeleton regions we've been getting since the jump to true 3D.

Either way, I fuckin' miss optional dungeons, shit like Mt. Mortar

You can only place blocks after obtaining the National Pokedex, which happens after entering the Hall of Fame.
