I don't know anything about the tournament episode but I keep seeing people say that Leon defeated Lance and that other champions were in the tournament as well, making Leon the strongest of the lot. Is that legit? I honestly can't imagine them doing that and having the implication that Steven, Cynthia, Alder, and Diantha were completely off screened. That sounds too awful to be true, especially with how much the anime (and honestly the franchise as a whole) goes out of its way to hype up Steven and Cynthia in particular.

Some people confusing their own perceptions of Lance with how he is meant to be perceived. Lance being the champion in Gen 2 is completely fitting; he is built up as worthy in his own right and it's not just a matter of the two only trainers who could challenge him not wanting to fill the seat.

Given all this, it's easy to see why Lance is positioned as numero dos. He is the champion that would make Leon look his best by defeating.

No, GameFreak never planned since she was only part of the plot and nothing more. The popularity for Lillie took them by surprise and eventually this make them take decision (as change some part of the plot in the remake).
