They had already changed Chikorita and Meganium to more closely resemble their final versions but hadn't got around to the middle stage yet. It's too different to those two to have been the actual middle stage for them.

The mod evolution is the remnant of a prior line of designs that was mostly replaced. Here's an example; Pikachu in Gen 1 didn't always loo the way he did. There's an interview somewhere describing how Pikachu's original design was to look like a type of ice cream (I think) desert, and the black tips on his ears are remnants of that. There's probably an even earlier green build, before the Gorochu line that had that weird desert based Pikachu.

Original chikorita was more of an amorphous blob, with basically only the current chikorita's head, so that middle stage would have fit it more.
