He (and all YouTubers) fake the reaction for the thumbnail before the stream starts, so that they have a thumbnail to use. Then, after the stream actually happens, they edit the thumbnail with their genuine (or "genuine") reaction. He himself has said it's stupid, but "everybody does it" (his words), which he's correct, but still.
