Retard. Johto was programmed first, it filled the cartridge, some optimization was done and then Kanto could be added. Jotho wasn't sacrificed for this.

There was never a time where Johto was intended to be on its own. They were going to cover all of Japan at first including a mini-Kanto, then they reduced the scale of the region to focus on just Kansai (Johto) and Kanto.

If anything, Johto was expanded because of it, since the only parts of it originally planned were New Bark, Violet, Goldenrod, Cianwood and Mt Silver (Ruins of Alph and Slowpoke Well existed too, but were located outside of Johto).

It's also worth mentioning that Cianwood has nothing to do with Kansai in reality and wasn't going to be part of Johto at first until late in development when they wanted a place for the freshly created Lugia (invented for the second movie. The original Silver mascot was likely a lion beast who got turned into the legendary trio) to live and for there to be a point to including Surf and Whirlpool as HMs.
