All we know is that he's a schizophrenic and nobody's looking after him, and he'll almost certainly kill himself if Leaf tells him to.
Hopefully he'll make an announcement before he does it so someone can call 9-1-1. That's pretty much the only way he'll get help since he's already an adult.and he actively refuses help.
