Being low on HP does mean your getting hurt, it doesn't mean your dying or anything that drastic (unless you're pushing past your limits), your just injured/harmed enough to a point where you need to rest and can't resist the Pokeball's capture net.
And also anime does count, why do you think it doesn't? It displays Pokemon very commonly injured after battles, the health care system for Pokemon is incredibly impressive and more or less takes perfect care of the harmed individuals.
