It's got a Pokemon Centre so its a city. More people pilgrimage through it than live in it but that still makes it a buzzing metropolis.

Early 2D games were an abstraction of a city or town due to hardware and programming limitations, BW was the first one to bring a massive town to life and even then it's pretty hollow.

It's a video game abstraction where things unimportant to the player are left out. The city is much bigger in "reality".

Is there any hack of the SpaceWorld ROM that's actually playable outside debug mode/has this kind of tileset? I've played Gold '97 which is great but it still wasn't enough.

Hardware limitations absolutely required the towns to be shrunk down. That's the entire reason RPGs dating back to the original Dragon Quest use isometric chibi overworlds.
Programming limitations, is a bit more iffy, but Game Freak being Game Freak sets a hard stop at what you can actually do since they've been incompetent since fucking Mendel Palace.
