Why are both main characters always of equal height? Men are supposed to be taller than women, while girls are supposed to be taller than boys.

for MCs that are meant to be 10, it's not out of the ordinary. For MCs implied to be older like Hilda and Hilbert, then yeah the guy should be taller

Those are averages, not mandates. The real reason is it's easier to model though, both in terms of making two characters out of the same basic model and making clothes fit on both characters when customization comes into play.

It's not like it's some unbreakable rule, there are plenty of teenage boys and girls who are the same height

Only at that point where the girls have hit puberty but the boys haven't.
The characters' ages are ambiguous in general, but they could either be pre-pubescent, or at that point where the boys have just caught up to the girls but haven't outgrown them yet.
