Whatever the reason, they mostly do it wrong and, worst is, the blame isn't on just on one hand, it could be on the higher ups thinking only in terms of money and selling timing, on the programmers being lazy or incompetent, on the players themselves saying they want things and then they complain because it's not what they imagined, etc...
